Indeed, a very sad incident. I cannot think of any reason why this needed to be done live. There are still entirely too many people in the electric trade who consider live work a necessity when it will save time or money. “If I don’t work on this live, the boss / customer will find someone who will”. Maybe someone felt pressured or maybe someone was just lazy. Aside from the human cost, someone just won the reverse lottery.

MSHA report

MSHA usually produces a very thorough report, but it takes time. Notice the work was being done on 10-10-10, which was a Sunday. Most quarries I have been in are closed on Sunday, only mentioning it because it suggests live work could have been avoided.

I searched the web for the contractor name and quickly read more, but just newspaper accounts which were not instructive.
