If a breaker fails or there is a phase to phase or phase to ground failure within a junction box or within an MCC- If the door stays closed and there was no danger to personnel- From a Regulatory reporting perspective- would you consider this an arc flash incident?

I know of no requirement to report an electrical fault to any government authority if there are no injuries.

I don't know of any requirement either. If it gets to the point where we have to report every component failure, we are in big trouble.
