In the UK it is a legal requirement to report an arc flash incident where there is no injury whatsoever. This is under the Reporting of Injuries and Dangerous Occurences Regulations. Basically any electrical flashover that results in equipment being out of action for more than 24 hours will need to be reported in which case there is a good chance that the authorities will investigate it. I suspect that this not the case in the US where no people are injured. Is this correct?
