I recall seeing an IEEE published article from way back, probably 1960's or 1970's vintage, discussing the subject of burndown and high resistance ground failures which operate undetected.

Does anyone know where this is at, article title, or other pointers?

Not sure if this is the one you were looking for but it was pretty good and from that time frame. I was at Square D when Harry was the Chief Engineer. He conducted much of the research in this area.

Beeman's handbook om power systems covered this but it is probably out of print.
