As far as I know there is no existing standard out there which recommends what goes in a arc-flash study/fault current study report. However, the IEEE 1584 group has a working group that is currently working on a new standard which will answer questions just like yours. As far as the report goes, it is pretty much defined by the scope of work. I have been working on an arc-flash report myself for couple of months and I could say the report should have a
--Executive Summary (Summarizing results of SC study, Coordination Study, AF study etc.)
--Introduction ( You can define the scope of work, objective, system description, background info on arc-flash for non-technical personnel.)
--Conclusions and Recommendations (after analyzing SC study, Coordination Study, and AF study results. Recommendations can be made on a cost of implementation basis)
--Incorporating Results

, you might want to add extra sections. Did I mention the assumptions list? Have a table of assumptions ready at the end of study...this is critical.
