This week's question is a follow up to last week. How long ago (if ever) did you perform your first lockout/tagout?

Low Voltage i,e, 480 volts= This past Sunday. High or medium voltage i,e, 14,475 volts= two weeks past.

44 years ago! Had to supply my own lock & the tag was a piece of cardboard torn off a discarded box. Working temporary power on an oil refinery expansion. I was a first year apprentice connecting welding machines into 60 amp 600 volt disconnects under the watchful eyes of my journeyman. There were no site requirements to lock out anything & no lockout procedure but our foreman insisted on it. Most electricians on site carried their own locks.

Question is FIRST lockout/tagout, not most recent.
