NFPA 70E 110.5(J) Incident Investigation states:
The electrical safety program shall include elements to investigate electrical incidents.

This weeks question (last one for 2020 before we wind down for the holidays)

No
Yes - Near Miss
Yes - Minor Injury
Yes - Major Injury
Yes - Fatality

Lineman didn't wait for colleague one span away to open tap prior to making contact with primary. He's permanently disabled. Please be safe out there.
