I am basically self employed, so it is my responsibility (to make sure my wife does it correctly).

It could be the Outside service or the Individual. If the Individual is responsible then they should have proper training on the care of the PPE.

The Employer is responsible, however we have that performed by an outside service, as well as I have sent helpers to the local laundrymat for the day to get some immidiate problems corrected.

I've seen companies do a better job having there floor mats cleaned than the electrical PPE. This has been a learning curve for us getting this established, as well as adaquate quantities of PPE to get the electricians to relinquish it for cleaning.

Talked with a large company that gave their employees an allowance for laundering their own PPE. A union concession. Who knows how or if they can enforce it.
