This one is simple but there is an underlying reason for the question (committee work)
Do you/company/client/consultant use a data collection/checklist form? Yes No N/A

Only if I don't have a one-line prior to survey.
I like to at least build the model off the existing single line diagram prior to surveying if it's available. Even better if I have shop drawings on the equipment and a power plan.
Field survey is mostly verification that I mark right in/on the model.
If neither are available and I have to collect all the data before modeling anything I have a form on my iPad for data collection with hard copy forms in my backpack as a backup plan.
Edit: it also depends on the size of the job. Very large models can be a little unwieldy on scene so sometimes forms are better depending on the amount of survey needing to be done.
